A medical or health care power of attorney is an advance directive that names the person assigned to make decisions for you when you are unable to do so. Some states call this directive a durable power of attorney for health care.

Choosing a person to act as your health care agent is important. Even if you have other legal documents regarding your care, some situations can’t be anticipated. You should choose a person who meets the following criteria:

• Is willing to discuss medical care and end-of-life issues with you
• Meets state health care agent requirements
• Can be trusted to make decisions that adhere to your wishes
• Can be trusted to be your advocate if there are disagreements about your care
